Declaration

• Shall observe and comply with all rules, regulation abd guidelines set by MAX SPORTS ACADEMY (MSA).
• Shall not interfere with any training session or facility.
• Authorize my minor child to participate in the chosen training session.
• Authorize MSA to use photographic or video images of my child for promotional or information purposes in brochures, flyers, creative, posters or on MSA website at no cost.
• Undertake to provide a fitness certificate (not more than 10 days old) stating that he is fit to complete in competitive training/exercise and can participate in the matches and that he doesn’t have any contagious disease illness.
• Authorize MSA to transport my child in a personal vehicle in the event of any medical emergency and discharge MSA of any or all liability.
• Authorize MSA to give any and all necessary authorizations in lieu of myself, for any and acts of treatment which may be decided by medical profession in the event of my child being the victim of an accident or illness. I, undertake to bear full financial responsibility incurred in that.
• Understand that fees once paid is non-refundable & non-adjustable.
• Agree to general terms and conditions for the academy course and undertake to pay the fee in advance/when due.
